No one should face barriers to their health

86Borders is a care coordination and member engagement company working for health plans. We reach the members a plan cannot reach on its own, and we walk with each of them until their care needs are met.

Why we exist

Most members who miss a screening or a refill are not refusing care. Something specific is in the way, and nobody has asked what it is.

We exist to ask, and then to take the barrier down: arrange the ride, find the doctor, speak the language, call the daughter who keeps the calendar. One member at a time, with a person on the other end of the line.

The barriers we work against

What members face

Language barriers, fear of a screening, unstable housing, not enough food, no ride, a primary care doctor who retired or moved. Each one is ordinary, and each one is enough to keep a gap open.

What the system adds

Outreach that is measured in attempts rather than results. Contact data that is out of date. Plan, clinical and pharmacy data kept in separate places. Call centers that never meet the member twice.

Our mission

To make sure no one faces barriers to their health. We do it with local Care Coordinators who build real relationships, and with ConnectAllCare, the platform that gives them the full picture and tracks every need to resolution.

Our values

Five values we hire and work by

They show up in how a coordinator opens a call and in how we report back to a plan.

Where we work

Our offices are in Atlanta, Georgia and Nashville, Tennessee. Our Care Coordinators are locally based, close to the communities they serve.

Named one of Modern Healthcare’s Best Places to Work in Healthcare for 2026.

Good coordinators stay when the work is done right, and continuity is what members feel.

Work with a team that stays until the need is met

If your plan has members it has not been able to reach, we would like to hear about them.
